CMS Caregaps and Quality
Caregaps reveal the delta between payor/provider performance and the established ideal or standard. We are building a world class modular caregaps framework that can automatically ingest many different kinds of payer data along with combining that with EHR data from sources like EPIC to present a real time view of caregap performance to providers and operational leaders. This is already being used inside our portfolio.
HCC Recapture, Opportunities and Suspects
We are building a modular framework for risk based contracts like ACO Reach to help with all things HCC, This includes HCC recapture, detecting opportunities, better care management through opportunity aggregation and classification and much better cost management for the practices. This is an important pillar for our portfolio companies as part of their progress towards Value based contracts.
